Need help with your Oregon Health Plan (OHP)? Salud Connect is here to help! We can assist you with applying for or renewing your Oregon Health Plan, reporting changes, and understanding your benefits. We also help connect you to care by scheduling appointments with primary care providers, dentists, vision providers, and other healthcare services.
If we are unable to directly provide the services you need, Salud Connect can connect you with trusted community partners and organizations that may be able to help. We have built strong relationships with local resources and are committed to helping you find the support and services that best meet your needs.
